Vein enlargement in the scrotum is the sign of varicocele, it can cause low sperm production and decreased sperm quality. Varicocele is the same as varicose veins that mostly occurs in the leg or feet. It can cause male infertility.

Symptoms:

Swelling and pain in the scrotum

Dull ache or heaviness in testicles

Lump in testiclesk

Unusually large or twisted veins

Fertility problems

Causes

The actual cause for developing varicocele is not known till now, but some research says it can be developed due to improper blood flow. Your spermatic vein carries blood to and from the testicles and if the valves present in veins are not properly supplying blood to testicles can cause vein enlargement and leads to varicocele.
